Organic germanium consists of a ring of 6 germanium atoms, which are tightly surrounded by 12 oxygen atoms.

The germanium is like an adhesive that holds the 12 oxygen atoms together in a very small space, but does not itself react with the organism.

This oxygen bound by the germanium is not released into the organism, but mainly serves as a radical scavenger for H + ions. Food is burned by the body, producing carbon dioxide (CO2) and hydrogen (H2). The carbon dioxide finds its way out of the body by exhaling it through the lungs.
